Welcome to Perrysburg Schools Enrollment Department!

Enrollment for 2022-23 School Year, (8/16/22 - 5/25/23)

Students must live within the Perrysburg School District attendance area with their custodial parent/guardian in order to be eligible for enrollment for Grades K - 12. Perrysburg Schools does not offer Inter-District Open Enrollment.


*Children must turn 5 years old by August 1, 2022, to attend kindergarten in the 2022-23 school year.
 
ENROLLMENT PROCESS
Perrysburg Schools has a two-part enrollment process - Completion of an online application, which is followed up by an in-person appointment at our central office to turn in enrollment paperwork and receive an overview of the district. Depending on your student’s grade level or prior school setting, a math/reading assessment will also take place at the same time (for Grades 7-12 and/or students coming from a homeschool environment where a report card is not obtainable).

Please review the instructions below, then click on the KG - Gr 12 Link above to begin enrollment.
  1. Create an account and enter your child’s information.
  2. Submit your child’s enrollment information.
  3. Click on the online calendar link to select a time to meet with enrollment staff in-person at the District's Central office on 140 E. Indiana Avenue in downtown Perrysburg. 
  4. Appointments for Grades K-12  are held at the Perrysburg School District Central Office (Commodore Building), at 140 E. Indiana Avenue. Parking is available on East 5th Street where you can also access the Visitor Entrance.
  5. If you are enrolling students in multiple grades, please make just ONE appointment time and all the children in your family will be taken care of at the same time.
REQUIRED DOCUMENTS
The following items will need to be presented at the appointment for copies to be made. Your child is NOT considered “enrolled” until ALL items are received.
  1.  ORIGINAL Photo ID and Child’s Original Birth Certificate or Passport with you even if you uploaded the documents in advance. Photo I.D. of Custodial Parent/Guardian must be presented in-person when registering.   Also, any paperwork showing a parental name change (i.e. divorce/re-marriage) must be presented.
  2. ORIGINAL Certified Birth Certificate (or passport),
  3. Proof of Residency. Most recent tax statement or print out from Wood County Auditor’s website or deed for homeowners; complete lease for renters. 
  4. Immunization Record (Immunization records from the student’s birth to the present must be submitted. If you do not have a copy, you may ask your child’s doctor to print a copy for you to bring to your appointment or ask them to fax us the records directly at the number below.)
  5. Recent copy of student’s recent report card/transcript. 
  6. Court file-stamped Custody Papers, if applicable to your child.

  1. Create an account and enter your child’s information.
  2. Submit your child’s enrollment information.
  3. Building Secretary Josie Holbert will contact you directly to schedule an enrollment appointment.
    Appointments for preschoolers are held at the Perrysburg Preschool located at 102 E. Broadway in Maumee.
