Welcome to Perrysburg Schools!
Students must live within the Perrysburg School District attendance area with their custodial parent/guardian in order to be eligible for enrollment in Grades K - 12. Perrysburg Schools does not offer Inter-District Open Enrollment.
 
2024-25 School Year Registration Dates
January 17, 2024: Online Pre-registration for Preschool Opens
January 22, 2024: Online Pre-registration for Kindergarten Opens
April 1, 2024: Online Pre-registration for Grades 1-12 Opens

**Please do not pre-register for 24-25 until the window opens on the dates above or students will not be in the correct school year and online forms will need to be redone.

  3. Once all online forms have been completed and signed, the parent/guardian will be contacted via email to schedule an appointment to complete enrollment. Enrollment appointments for Grades K – 12 are held at 140 E. Indiana Avenue.
Required Documents
  1. Parent/Guardian Driver's License or State Issued ID Card - Must be presented at appointment for proof of identity. Also, any paperwork showing a parental name change (i.e., divorce/re-marriage) must be presented.
  2. Child's Birth Certificate or Passport- An original (or certified copy) of student's Birth Certificate must be presented at registration.
  3. Immunization Records- Complete immunization record.
    • Preschool Only – Preschool Medical Statement. Medical Statements are due no later than 30 days after date of admission and every 13 months from the date of.
    • KG Only - Physical dated within one year from start of school (doctor can use any form). These forms are due by the first day of school and can be submitted directly to the building of attendance.
  4. K-12 Grade Card/Transcript - Current/most recent report card/progress report or copy of transcript. If no record is provided, student enrollment may be delayed until grade level can be verified with previous school.
  5. Special Education Paperwork - (if applicable) The current ETR/MFE and IEP for students with disabilities, or the current Section 504 Accommodation Plan, or Gifted Identification information should be presented at the time of enrollment, if applicable. An official request will be made to the previous school for these documents as appropriate; however, having the documents upfront facilitates a smoother transition.
  6. Custody Documents - (if applicable) If the child is not living with both biological parents, then you must provide a court file-stamped custody document. NOTE: Ohio law requires the entire custody document be on file with the school.
  7. Proof of Residency - Provide one item from List A and one item from List B of the residency documents on the website. The parent/guardian of the student(s) being registered must be listed on documents; otherwise, a Residency Affidavit will need to be completed – See “Living with a Perrysburg Resident.”
