New Student Enrollment Information for Grades K-12

Parents/Guardians who are Perrysburg School District residents with new students, please click on this link to pre-register for the 2019-2020 school year:
After parents/guardians complete the New Student Enrollment, they will be asked to schedule an enrollment appointment, which will be held at Central Office (140 E. Indiana Ave.). Parents/Guardians will schedule an enrollment appointment from the submission confirmation page of the enrollment form, using the link provided at that time.

Note: Parents/Guardians need to complete the forms online and schedule an appointment time to be considered pre-registered.

Parents/Guardians should plan to bring to the scheduled appointment the required documents (certified original birth certificate, photo ID as the parent/guardian, proof of residency - see “New Student Enrollment” tab for detailed description by selecting the link), immunization record, and complete court file-stamped custody papers (if applicable).

For New 7th - 9th Grade Students ONLY:

The student will need to take the STAR Math and Reading Test. This is a test for enrollment that is done on the computer and will take about 45 minutes. The STAR Reading and Math Test results are to aid the school in your child's proper class placement to ensure their success at Perrysburg. This test may be done at the time of the enrollment appointment or scheduled at a later date if needed.
The following items need to be presented at the scheduled registration appointment. Copies will be made during that time. Your child is not considered “enrolled” until all items are received.
  1. Photo I.D. of Custodial Parent/Guardian must be presented when registering. Also, any paperwork showing a parental name change (i.e. divorce/re-marriage) must be presented.
  2. Proof of Residency.
  3. Certified Original Birth Certificate.
  4. Immunization Record (Immunization records from the student’s birth to the present must be submitted.)
  5. Court file-stamped Custody Papers, if applicable to your child.

In addition, it is recommended that you also bring the following to your appointment:

  1. Most recent grade card.
  2. Current transcript or most recent class schedule (High School Only).
  3. Special Education Records (IEP/ETR), if applicable.
  4. Written Education Plan (WEP), if applicable.
  5. 504 records, (evaluation/plan) if applicable.
  6. Parent Request and Authorization to Administer a Prescribed/Over the Counter Medication or Drug if your child will need to receive medication during school hours.

In order to accommodate your registration quickly, we MUST comply with the legal requirements of the following:

  • Federal/State Law - “Missing Child Act”
  • State Law ORC 3113.64 – Legal Custody
  • State Law ORC 3313.64 - Legal Residency
  • Perrysburg Board of Education Policy Guidelines #5111
