Proof of Residency Requirements

Perrysburg Exempted Village Schools provides education for children meeting residency requirements. To be eligible for public education, a student must be in the legally recognized custody of a parent/guardian or government agency residing within the district. Perrysburg Schools does not offer Inter-District Open Enrollment.

 

To enroll your student in the Perrysburg Exempted Village School District, or if you move within the district, you must provide the proper documentation to establish residency. The parent/guardian of the student(s) being registered must be listed on residency documents; otherwise, a Residency Affidavit 5111 F2 will need to be completed – See “Living with a Perrysburg Resident.”

PLEASE SUBMIT

ONE Document from List A     AND       ONE Document from List B

The name and address on these documents must match the name and address of the parent/guardian of the student(s) being registered.

 

If the parent/guardian cannot prove residency with a document from List A and is living with a Perrysburg Resident, a Residency Affidavit 5111 F2 will need to be completed, including providing supporting documentation. See “Living with a Perrysburg Resident.”

  • Property Owner Listing from Wood County Auditor's Website
  • Deed
  • Recent Property Tax Statement
  • Current Mortgage Statement
  • Purchase Agreement w/Possession Date**
  • Building Contract w/Estimated Completion Date**
  • Rental/Lease Agreement**
     - Complete copy of unexpired, signed lease agreement MUST be provided in its entirety and contain terms, property owner’s name, address, phone number, and signatures of lessor and lessee. 
  • Letter signed by landlord, including his/her phone number and signature – valid for one year.
  • Utility Bill
  • Bank Statement
  • Pay Stub
  • W-2
  • Driver’s License w/Perrysburg Address
  • Car or Medical Insurance Statement
  • State or Federal Mail
  • Voter Registration

 

** Students enrolling with residency documentation containing a future occupancy date may be enrolled for a period not to exceed ninety (90) calendar days prior to move-in. An item from List B should be provided within 30 days of occupancy.



Living With a Perrysburg Resident 

According to Ohio’s School Attendance Law (O.R.C. 3313.63), a student can only attend school tuition-free where his/her custodial parent/guardian resides. The Residency Affidavit 5111 F2 must be completed and signed by both the parent/guardian and the Perrysburg homeowner/leaseholder and notarized whenever a parent cannot verify that he/she owns or rents a residence in the district. Supporting documentation to be turned in with the affidavit is listed below. 

 

In addition to the notarized Residency Affidavit 5111 F2, provide copies of the following supporting documents. The Residency Affidavit is valid for one year.
