Pre-Registration for 2025-26 Preschool begins Jan. 15

Perrysburg School District Residents who are parents/guardians of *new incoming 2025-26 preschool student(s) may click here to pre-register beginning Wednesday, January 15, 2025
 
Students must be three years old and toilet-trained to begin preschool.
 
The school district uses web-based software called FinalForms for enrollment and updating of student information from year-to-year. New families will create a FinalForms account, while families with other children already attending school in Perrysburg Schools will be able to log in to their existing FinalForms account and click “add a student” to enter their child’s information.
 
We encourage parents and guardians to register online as soon as possible once the pre-registration window opens since spots are limited.
 
Once the online form is submitted, you will be contacted by the Preschool with instructions to complete the registration.
 
Parents/guardians, to complete your registration, you will need to provide the required documents and forms:
 Certified original birth certificate
 Photo ID as the parent/guardian
 Proof of residency - see “New Student Enrollment” tab for detailed description
 Immunization record
 Entire court file-stamped custody papers (if applicable)
 Completed tuition form (form will be emailed to you after enrollment is complete; the form must be returned to our Fiscal Office to secure placement for next school year)
 
*Parents/guardians of CURRENT Perrysburg Preschool students at Union Preschool DO NOT need to pre-register if returning to Preschool for the 2025-26 school year. Current Union Preschool Parents/Guardians will be asked to verify their information in FinalForms prior to the beginning of the school year.
