7th and 12th Grade Immunization Update

Update for Parents and Guardians of 7th and 12th grade students for the 2016-2017 school year:

The Ohio Department of Health requires that schools have a record of completed immunizations for every student on file no later than two weeks after the start of the school year. We are quickly approaching that date and we still have a number of 7th and 12th grade students who have not met the following requirements:

Beginning with the start of the current school year, all students entering 7th and 12th grade were required by the ODH to have received a meningococcal (serogroups A, C, W and Y) immunization. One dose is required prior to entry into the 7th grade. A second dose is required prior to entry into the 12th grade. The second dose must be administered on or after the 16th birthday with at least eight weeks between the 1st and 2nd doses. If the first dose of the meningococcal vaccine was administered after the 16th birthday, a second dose is not required.

In addition, all 7th graders were also required to have a Tdap vaccine prior to the start of school.

If your student has not completed these immunizations yet, they can be obtained through the student’s personal physician and/or some local area pharmacies. Please make arrangements to do so immediately and provide the building nurse with documentation from the vaccine administer when the vaccine was given.

Any questions can be directed to Debbie Reddick at the high school – 419-874-3181, ext 5010, or Jodi Perales at the junior high school – 419–874-9131, ext 4011.
