Immunization Requirements

Attention Parents and Guardians of Current 6th and 11th Grade Students:

Per Ohio State Law, all students entering 7th and 12th grades are required to have received a meningococcal (serogroups A, C, W and Y) immunization prior to the start of school. One dose is required prior to entry into the 7th grade. A second dose is required prior to entry into the 12th grade. The second dose must be administered on or after the 16th birthday with at least eight weeks between the 1st and 2nd doses. If the first dose of the meningococcal vaccine was administered after the 16th birthday, a second dose is not required.

In addition, all 7th graders are also required to have a Tdap vaccine prior to the start of school.

Please make plans to get these immunizations completed before the start of the 2017-2018 school year. When completed, please turn in to the building nurse documentation from the vaccine administrator verifying the immunization given, the student’s name and the date it was administered.

Immunizations can be obtained from the child’s personal physician, through local pharmacies or through Lucas County Shots 4 Tots N Teens.
