Visitors are not permitted the week prior to a school vacation to begin nor during the first and last week of a semester. Student visitors are welcome as long as they conduct themselves properly as guests and follow all rules and regulations pertaining to students at Perrysburg High School. It is the responsibility of the host student to be sure their guest knows what is expected of him/her. The host must also remain with the visitor at all times, including lunch and rest room visits.

To qualify as a guest to P.H.S. requires the following: (Check one of the boxes)

1. The visitor must be of the same sex as the host.
2. The visitor must be a high school student with permission from his/her home school to be absent and be:
   • a student attending a private high school living in the Perrysburg school district and planning on attending P.H.S.
   • a student from an area high school whose parents are moving to Perrysburg or a part of a student inter-school exchange program.
   • a foreign exchange program, educational purpose
3. Host parents must have requested at least five days in advance from their son/daughter to bring a guest to school.
4. The host must receive permission from all his/her teachers to bring a guest to class.
5. The host must accept full responsibility for his/her guest's behavior during the visit.
6. Social visits are not acceptable.

COMPLETE THE INFORMATION BELOW AND RETURN TO THE HIGH SCHOOL OFFICE FIVE DAYS PRIOR TO THE DAY YOU ARE REQUESTING TO BRING A VISITOR!
