INJURY TO FACULTY

If a faculty or staff member is injured while on the job he/she should complete an Employee Accident Report. This should be done as soon as possible after the injury so the dates and circumstances are easily recalled and verified in case it should become necessary for Workman's Compensation or insurance purposes.

What To Do If You Are Injured At Work.

If you are injured at work and it is an emergency:

  1. Contact your supervisor and seek immediate medical attention. The initial visit is always covered for work related injuries.
  2. As soon as possible after receiving medical attention, see your supervisor and complete an employee accident report
  3. Call Gates-McDonald at 1-800-642-7587 and they will tell you how to proceed. This step is imperative if you will be filing a worker's compensation claim.

If you are injured at work and it is not an emergency:

  1. Notify your supervisor
  2. Complete an employee accident report
  3. Contact your building secretary or supervisor for an Ohio Worker's Comp ID Card.
  4. Call Gates-McDonald at 1-800-642-7587 to receive authorization and get referred to an approved worker's compensation provider or clinic.
  5. Go to the designated provider or clinic and present your ID card.
  6. Return the ID card to your building secretary or supervisor

INJURY TO STUDENTS

If a student is injured while under your supervision you should complete a Student Injury Report immediately after taking care of the student. This report should be completed even if the injury seems minor at the time in case of unforeseen complications. Be sure to indicate any treatment you might have done to the injured student.

 

STUDENT ILLNESS

The clinic is provided for students who become ill at school. If a student becomes ill during the day, he/she is to ask the classroom teacher for written permission to go to the clinic, located in the guidance office. Students should not report to the clinic without first receiving permission from their teacher. If the teacher needs help getting the student to the clinic he/she should contact the office via the P.A. and inform the office of the need for assistance. Be sure to request the wheel chair if needed.
