FIELD TRIP REQUEST PROCEDURES

 Field Trips will are not to be requested during the

final 5 teaching days of a quarter.

l. Teachers must submit an Educational Field Trip Request the following information at least 10 days in advance of the field trip. Forms are available from the principal's secretary.

 

2. The teacher will publish the list of students for all teachers a minimum of five (5) teaching days in advance of the trip. The list will be distributed via email.

 

3. The teacher will distribute and collect the Field Trip and Emergency Medical Authorization sheet. Students who do not return the emergency medical sheet will not be permitted to participate in the field trip.

 

4. Teachers must alphabetize the Emergency Medical Forms and place them in an envelope to protect them from loss. The outside of the envelope must contain the school name and address; date of the trip; and the destination.

 

5. Teachers must provide the security and supervision necessary to insure the health and safety of the students and to enforce the school rules and regulations.

 

6. Teachers are encouraged to use only school or adult driven transportation. If it is absolutely necessary to use student drivers, parents must be informed prior to their signing the permission form and it must be understood that the student's insurance will be responsible for any accidents he is involved in.

 

7. Teachers are to make transportation arrangements by completing a Bus Transportation form . If nine or fewer passengers will be going the school van may be used if the teacher is an approved driver and completes the Van Trip Request.
