EMERGENCY MEDICAL AUTHORIZATION FORM
All student are required to have a properly completed Emergency Medical Authorization form on file in the office. These forms by law must be new each school year and must accompany the student if he/she leaves the building on a field trip or other authorized trip sponsored or supervised by school personnel. This form gives the person in charge instructions as well as authority to help the student in the case of medical emergency. No student may attend school without an Emergency Medical card on file in the office.
